Consent to Operate

 Following documents  are required to be submitted/uploaded by the applicant at the time of applying for  Consent to operate (Fresh/ Renewal) :-

Checklist:-  

A)Consent  to Operate (Fresh) 

i) Application form Sch-I, Form-I as per Hazardous Waste Rules( in case of industries falling in Hazardous  category)duly filled in and signed by proprietor / authorized representative.

ii) Copy of last consent issued.

iii) Consent fee as per prescribed by the Board.

iv) Land papers /lease deed in case of unts within industrial  area if not submitted earlier.

v) Copy of PMT registration /DIC assessment.

vi) Photograph of PCD/ETP installed.

(Note :- PMT /DIC assessment and SMR to be submitted after trial run of four months to central office directly.)

B) Consent  to Operate (Renewal)

i) Application form Sch-I,Form-I HWA and Form-IV of Annual Return,  in case of industries falling in Hazardous Category duly filled  in and signed by Proprietor/authorized representative.

ii) Copy  of previous consent to operate issued by the Board.

iii) Consent fee as per prescribed by the Board.

iv)Copy of SMR
v) Self Declaration on compliance of previous consent conditions imposed  in earlier consent sissued by the Board. 

All the documents as given above are to be submitted in triplicate.
